What to Expect from Anti-Wrinkle (Botulinum Toxin) Treatment: A Complete Guide

“Will I still look like me?”

The short answer is yes.

Modern anti-wrinkle treatments aren’t about making everyone look the same. They’re about helping you achieve the look you want. For many people that’s a refreshed, natural appearance which retains movement; for others it’s a beautifully smooth forehead with very little movement at all. The important thing is that the result is tailored to your goals, not someone else’s.

In this guide, I’ll explain exactly what botulinum toxin treatment is, how it works, what to expect before and after treatment, how long results take to appear, and the potential risks and side effects.


What Is Botulinum Toxin?

Botulinum toxin is a purified protein that temporarily relaxes selected muscles.

It has been used safely in medicine for over 30 years to treat a wide range of conditions including migraines, excessive sweating, muscle spasms, eye disorders and facial lines.

In aesthetics, it is most commonly used to reduce the appearance of dynamic wrinkles—the lines that develop when we repeatedly make facial expressions such as frowning, squinting, smiling or raising our eyebrows.

How Does It Work?

Every time you smile, frown, squint or raise your eyebrows, your nerves send signals telling the muscles of your face to contract.

Botulinum toxin temporarily blocks these signals.

As the treated muscles relax:

  • Existing lines become softer.
  • Skin appears smoother.
  • New lines are less likely to deepen.

Importantly, the treatment does not affect sensation. Your skin will feel completely normal.

Think of it as turning down the volume rather than switching the music off completely.


What Happens During Treatment?

Treatment itself is surprisingly quick.

After discussing your concerns and examining your facial movement, a series of tiny injections are placed into carefully selected muscles.

The needles used are extremely fine, and most patients describe the sensation as:

  • A small pinprick
  • Mild stinging
  • Less uncomfortable than a blood test

Most appointments take around 20–30 minutes, including consultation and photographs.

You can usually return straight to work, school runs, meetings or whatever else your day has planned afterwards.

When Will I See Results?

One of the biggest misconceptions is that anti-wrinkle treatment works immediately.

It doesn’t. The following timeline will help you understand the journey!

Day 1–3

You may notice tiny bumps at the injection sites for around 15–30 minutes after treatment.

Most people look completely normal by the time they leave the clinic.

Day 3–5

The treatment begins to take effect.

You may notice movement starting to reduce. This may be uneven at first as muscle groups are affected at different rates – trust the process!

Day 7–10

Most patients can clearly see improvement.

Lines appear softer and facial movement becomes more controlled.

Around Day 14

Full results are usually visible.

This is why review appointments are generally scheduled around two weeks after treatment.

How Long Does Anti-Wrinkle Treatment Last?

For most people, results last between 3 and 4 months.

This varies depending on:

  • Individual metabolism
  • Muscle strength
  • Lifestyle
  • Exercise levels
  • Previous treatments

Over time, muscle activity gradually returns and movement slowly increases.

Many patients choose maintenance treatments every 3–4 months simply because they like the results—not because they have to.


Will I Look Frozen?

This is probably the most frequently asked question in aesthetics.

The honest answer is: it depends on the look you’re hoping to achieve.

Most patients tell me they want to look refreshed, well-rested and natural rather than obviously “done”. In these cases, the aim is to soften facial movement while preserving natural expression, so you can still smile, laugh, look surprised and show emotion.

However, aesthetic treatment is personal. Some people genuinely prefer a smoother, more immobile forehead with very little movement, and that’s perfectly reasonable too. During your consultation we’ll discuss exactly what you’re hoping to achieve so that your treatment can be tailored to your preferences.

The important thing is that your result shouldn’t be an accident—it should be a conscious choice.

Does Anti-Wrinkle Treatment Prevent Wrinkles?

Interestingly, yes.

Because treated muscles contract less forcefully, the skin experiences less repeated folding over time.

This can help slow the development of deeper static lines and wrinkles.

Many patients therefore choose treatment not only to improve existing lines but also as part of a longer-term skin ageing strategy.

Think of it as maintenance for your skin—rather like servicing your car before something starts rattling.


If I Start Having Anti-Wrinkle Treatment, Will I Need It Forever?

No!

This is one of the biggest myths surrounding anti-wrinkle treatment.

Having botulinum toxin does not commit you to a lifetime of treatment. The effects are temporary and, if you decide not to continue, the treated muscles will gradually regain their normal function over time.

Many people choose to continue because they enjoy the results and notice improvements in how they look and feel. Others may have treatment for a few years and then stop, maybe switch to another regenerative treatment such as skin boosters, polynucleotides or microneedling; or they may simply reduce how often they attend.

The choice is entirely yours.

Think of it a bit like having your hair coloured. Once you’ve seen the results, you may decide you prefer it and want to maintain it—but you’re certainly not obliged to continue.


Will My Wrinkles Be Worse When the Treatment Wears Off?

No—this is one of the biggest misconceptions about anti-wrinkle treatment.

As the effects of botulinum toxin gradually wear off, the treated muscles simply return to their usual level of activity. Your face goes back to behaving as it did before treatment. There is no rebound effect, and your wrinkles do not suddenly become deeper because you’ve had treatment.

In fact, many people find that their lines actually look a little better than they did before they started treatment. That’s because, for several months, the skin has had a break from repeatedly folding in the same places every time you frown or raise your eyebrows. During that time, the skin has had an opportunity to recover, and existing lines may soften.

So why do some people feel their wrinkles are worse? Usually, it’s because they’ve become used to seeing smoother skin. When movement gradually returns, the lines that were always there become noticeable again. It’s a bit like taking your watch off after wearing it every day—you don’t notice it’s there until it’s gone.

If you choose not to have any further treatment, your face will simply continue the normal ageing process from where it would naturally have been. Anti-wrinkle treatment doesn’t accelerate ageing, and it doesn’t leave you “worse off” for having tried it.


Will I Build Up a Resistance to Botulinum Toxin?

For the vast majority of patients, no.

In rare cases, some individuals can develop antibodies that make treatment less effective over time. However, highly purified products such as Bocouture® and Nuceiva appear to have a lower risk of antibody formation than some older formulations.

Following appropriate treatment intervals—typically every 3–4 months—helps minimise this already small risk.


What Happens If I Miss a Treatment?

Nothing dramatic.

You won’t suddenly wake up one morning looking older.

As the treatment gradually wears off, muscle movement slowly returns over several weeks. Any lines that were present before treatment will gradually become visible again. Some patients come religiously every 3 months and others come once or twice a year – it is entirely up to you (and your budget).

There is no rebound effect, no withdrawal and no sudden worsening of wrinkles.


What Are the Risks and Side Effects?

Like any medical treatment, botulinum toxin carries potential risks and side effects.

Fortunately, most are mild and temporary.

Common Side Effects

  • Small bumps at the injection sites
  • Mild redness
  • Minor swelling
  • Tenderness
  • Small bruises
  • Temporary headache

These usually settle within hours to a few days.

Less Common Side Effects

  • Minor asymmetry
  • Uneven eyebrow position
  • A temporary heavy feeling in the forehead
  • Temporary eyelid droop (ptosis)
  • Temporary brow droop

These effects are uncommon and generally improve as the treatment gradually wears off.

Rare Side Effects

Serious complications are extremely uncommon when treatment is performed by an appropriately trained medical professional following a full consultation and assessment.


What Should I Avoid After Treatment?

After treatment I recommend:

For the first 4 hours

Avoid:

  • Lying flat
  • Vigorous exercise
  • Facial massage
  • Excessive rubbing of the treated areas

For the first 24 hours

Avoid:

  • Saunas – this can increase bruising risk due to vasodilatation
  • Steam rooms
  • Hot tubs
  • Intensive exercise – there is a theoretical risk of moving the product to other nearby muscles
  • Excessive alcohol consumption – this actually reduces the potency of the toxin!

Normal washing, skincare and makeup can usually continue later the same day.


Who Is Suitable for Treatment?

Anti-wrinkle treatment may be suitable for adults wishing to:

  • Reduce facial lines
  • Prevent deeper wrinkle formation
  • Achieve a fresher appearance
  • Treat excessive sweating
  • Improve certain muscle-related concerns

Treatment may not be suitable if you:

  • Are pregnant
  • Are breastfeeding
  • Have certain neurological conditions
  • Have an infection at the treatment site
  • Have previously experienced a significant reaction to botulinum toxin

When will I see the results?

You may notice the first changes from around 3–5 days, with most patients seeing a significant improvement by 7–10 days. Full results are usually seen after approximately two weeks, which is why review appointments are typically arranged around this time.


Can I go back to work afterwards?

Yes. Anti-wrinkle treatment is often referred to as a “lunchtime procedure” because there is very little downtime. You may have tiny raised bumps for 15–30 minutes after treatment and occasionally a small bruise, but most people return straight to work or their normal daily activities.


Will I still be able to move my face?

Almost certainly—unless your goal is to have very little movement.

Most patients choose a natural-looking result that softens wrinkles while preserving facial expression. However, some people prefer a smoother, more immobile look. During your consultation we’ll discuss exactly what you’d like to achieve so your treatment can be tailored to your preferences.


Will people know I’ve had treatment?

Not necessarily.

Good anti-wrinkle treatment should enhance your appearance rather than change it. But if you are happy with your results – don’t keep it a secret, let’s bust the myth that wrinkle injections look obvious!


Can anti-wrinkle treatment stop wrinkles forming?

It can’t stop the natural ageing process, but it can help slow the formation of expression lines by reducing the repeated muscle movements that contribute to wrinkles over time.


Am I too young or too old for treatment?

There isn’t a “right” age.

Some people choose treatment in their late twenties or thirties to soften early expression lines, while others don’t start until much later. The best time is when your facial lines begin to bother you—not when you reach a particular birthday.


Can men have anti-wrinkle treatment?

Absolutely.

Yes. Is there anyone who shouldn’t have anti-wrinkle treatment?

Yes. Treatment may not be suitable if you are pregnant, breastfeeding, have certain neurological conditions, have an active infection at the treatment site, or have had a previous allergic reaction to botulinum toxin. A full medical consultation will always be carried out beforehand to ensure treatment is appropriate for you.


How do I know if anti-wrinkle treatment is right for me?
